How and why did you become a reproductive medicine specialist?<\/strong><\/p>\n\n\n\n<p class=\"wp-block-paragraph\">\u2014 When I was a student at Karaganda Medical Academy, life brought me together with a wonderful young family. We became very close, and they were also distant relatives of mine. I lived with them for more than two years.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">But they had one problem \u2014 they could not have children. Naturally, there were occasional disagreements within the family, and I understood that some of them might have been related to their inability to have a child.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">I saw them undergoing treatment and medical examinations from time to time. I wanted to help them. I began studying infertility using various sources and discovered that it was a fairly widespread problem, but one that people rarely discussed openly. They often could not talk about it at all; instead, it remained a deeply personal source of pain within the family and among their loved ones.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Because I was still a student, no one was coming to me for medical help, of course. But on my own initiative, I tried to find possible treatment options for them and see whether there was anything I could suggest.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">After graduating from medical academy, I decided to become a reproductive medicine specialist. Through the Bolashak program, I entered a clinical residency at the Moscow Medical Academy.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">When I returned, I submitted my application and r\u00e9sum\u00e9 to the first specialized infertility treatment clinic. At the time, I knew that it was headed by Saltanat Baikoshkarova.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">About a week later, I was invited for an interview. It lasted a long time, and we had a very interesting discussion. I was offered the position.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Afterward, I repeatedly traveled abroad for further professional training. As the effectiveness of our work improved, increasing numbers of patients came to us. We had opportunities to study modern treatment methods abroad, including in Europe and the United States. We attended international congresses virtually every year and introduced the experience and technologies we learned about into our work here.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Today, there are nine branches across Kazakhstan. But in 2007, the first branch was only just opening in Astana.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Thanks to the work of our team in Astana, more than 5,000 children have already been born. ECOMED itself has been operating for 23 years. Since 1995, more than 12,000 children have been born with our help. I consider this an enormous achievement.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">IVF as a method of infertility treatment emerged in the 1970s. The first child born through IVF has already grown up and had children of her own, conceived naturally. She was born in England, and her name is Louise Brown.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">\u2014&nbsp;<strong>How developed is reproductive medicine in Kazakhstan, and how competitive are our clinics?<\/strong><\/p>\n\n\n\n<p class=\"wp-block-paragraph\">\u2014 There are quite a few clinics providing assisted reproduction and infertility treatment \u2014 more than 20 \u2014 but not all of them are equally competitive.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">They also achieve certain results, but there are only three or four leading clinics that operate in accordance with current international requirements and standards.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">I would like to see greater government support as well as oversight of centers working in this field. I believe that our people deserve access to high-quality medical care.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">This field and the effectiveness of our work can only continue to develop if we constantly improve, learn, and grow.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">\u2014&nbsp;<strong>It has been reported that you are developing new and highly effective approaches to IVF treatment. Could you tell us more about your work in this area?<\/strong><\/p>\n\n\n\n<p class=\"wp-block-paragraph\">\u2014 One of the most important approaches that my mentor, Saltanat Baikoshkarova, and I have introduced is making treatment more accessible to patients.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">One such approach is mild ovarian stimulation \u2014 treatment with minimal stimulation and without a heavy hormonal burden on the body, as close as possible to natural physiological conditions.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">In some cases, we can retrieve oocytes without general anesthesia, fertilize them, and achieve results comparable to those of more complex and expensive programs.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">This is made possible by high-quality technologies, advanced air purification systems, specialized embryo culture equipment, culture media, and carefully controlled conditions for embryo development.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Today, there is not always a need to retrieve a large number of oocytes simply to transfer one or two embryos.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">There are gentler approaches for a woman\u2019s body and more affordable approaches financially that can still provide high effectiveness.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">I am currently also working on fertility preservation for cancer patients.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Cancer diagnosis and treatment have advanced significantly. However, chemotherapy and radiation therapy can often have a negative impact on reproductive function.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">As a result, men may lose their ability to produce sperm, while women may experience loss of ovarian function.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Preserving reproductive material before cancer treatment \u2014 for example, freezing oocytes, testicular tissue, or sperm \u2014 can allow patients to use their own genetic material to have children after recovery.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">We are actively working with these methods. We discuss this issue at international congresses and encourage oncologists and hematologists to become more involved in fertility preservation.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">\u2014&nbsp;<strong>You also help patients from Europe, the United States, and Arab countries. How often do international patients come to you?<\/strong><\/p>\n\n\n\n<p class=\"wp-block-paragraph\">\u2014 I would not say that medical tourism is highly developed in our country or at our center.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">However, we know many people from other countries who work in Kazakhstan, as well as many Kazakhstanis who have married and now live abroad.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">They know us and understand the capabilities of medicine in Kazakhstan. They are often the ones who return here for treatment, achieve successful results, and later recommend us to people they know abroad.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Many children have been born to our patients in Germany, the United Kingdom, and the United States. Patients also come to us from Africa.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">We frequently receive patients from neighboring countries, including Uzbekistan, Kyrgyzstan, Russia, and Tajikistan.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">\u2014&nbsp;<strong>How accessible is IVF treatment for patients, and how much do the procedures cost?<\/strong><\/p>\n\n\n\n<p class=\"wp-block-paragraph\">\u2014 In terms of cost, IVF in Kazakhstan is considerably less expensive than in many other countries.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The procedures are performed at a comparable level but cost significantly less than in Europe or the United States.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">This is despite the fact that medical supplies and equipment are manufactured much closer to clinics in those countries. We have to import virtually all of our equipment and supplies, which also involves customs clearance and other expenses.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">At the same time, income levels and salaries in Kazakhstan are not the same as in Europe or the United States.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Overall, the procedure here can be three to four times less expensive.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">At present, one IVF cycle may cost from approximately KZT 400,000 to KZT 1 million. Some of the methods we have introduced cost approximately KZT 300,000\u2013400,000.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">\u2014&nbsp;<strong>Almaz Kurmanaliyevich, how often do you stay in touch with your patients?<\/strong><\/p>\n\n\n\n<p class=\"wp-block-paragraph\">\u2014 Communication with patients is continuous.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">We continue seeing some families for years because, in certain cases, natural conception is completely impossible. As a result, their first, second, and third children may all be conceived through IVF.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Let me give you an example.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Eight years ago, we performed one IVF cycle for a patient. We retrieved 20 oocytes, from which eight good-quality embryos developed. We transferred one embryo into the uterus and froze the remaining seven.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">She became pregnant and gave birth to a daughter.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Later, the couple wanted another child \u2014 they hoped for a boy. We transferred another embryo, and they had another girl.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Three years later, they returned again, and this time they had a boy.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">From one perspective, all of this resulted from a single IVF cycle. These children were born several years apart, but all of their embryos were created on the same day.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">That is one of the major advantages of being able to create and cryopreserve embryos.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Patients may return five or six years later, undergo an embryo transfer, and become pregnant. The parents themselves may already be 40 years old, but the embryos retain the biological characteristics associated with the age at which they were created.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">At present, our center performs approximately 1,500 IVF cycles per year.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Practically every day, one or two children are born in Astana as a result of the work we do. Every day, I receive text messages or phone calls telling me that a boy, a girl, or twins have been born.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">\u2014&nbsp;<strong>We noticed a souvenir on your desk that appears to have been made especially for you. Which patients gave it to you?<\/strong><\/p>\n\n\n\n<p class=\"wp-block-paragraph\">\u2014 This gift was brought to me by patients who had already had a daughter and were undergoing another IVF program at the time.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">They were waiting for the pregnancy test following another embryo transfer. We had transferred two embryos into the uterus.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">During their first treatment, we transferred one embryo, and they had a child. This time, we transferred two.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Perhaps they somehow knew, or perhaps it was simply intuition, but they brought me a figurine depicting three babies.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">And indeed, the pregnancy test later confirmed that they were expecting twins.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">It is a very special and meaningful gift to me.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">\u2014&nbsp;<strong>How did you become a participant in the \u201c100 New Faces of Kazakhstan\u201d project, and who nominated you?<\/strong><\/p>\n\n\n\n<p class=\"wp-block-paragraph\">\u2014 I had heard about the project, but I had no intention of submitting an application myself. I did not think that I should apply or that I would become one of the winners.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">I still do not know exactly who submitted my nomination. I think it may have been the clinic management or perhaps one of my patients.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">I simply found out that I had been nominated for the project.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">It was extremely gratifying to learn that I had become one of the 100 people chosen through the public vote.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">I am very grateful to everyone who voted for me and placed their trust in me.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">At the same time, it is a great responsibility.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Everyone wants to be successful and to be a good person. Seeing the results of the vote made me feel an even greater sense of responsibility. I understand that I need to live up to this recognition and work even harder.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">I will continue striving to become a better doctor and a better person.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">That is my dream \u2014 to be a good doctor and to help people.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">When you genuinely try to help someone and do so wholeheartedly, patients can feel it.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">I understand that this is private medicine, but when patients see that you are acting conscientiously and sincerely want to help them, they become calmer.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">They begin to trust the process.
